Our Services

Forward Community Care Ltd provides high specialist Domiciliary Care and Supported Living Services for people experiencing complex mental health needs, difficulty with illicit substances and for those who have a forensic history. We also cater for individuals who have a functional mental illness who secondary diagnoses of a learning disability or autism.

We work in partnership with the clinical team to optimise mental health stability, physical well-being and recovery. The service aspires to minimise any potential risks to others by ensuring a comprehensive assessment process and an achievable support plan for the service users. The service aims to offer our residents the opportunity to live as independently as possible with the support of staff whilst maintain an optimal level of their mental health and physical well-being.

Domiciliary and Housing Support

  • Ensure people are supported to maintain their tenancy agreement and fill relevant paperwork and benefit forms so that they do not fall into arrears.

  •   Support people to obtain the right benefits they are entitled to.

  •   Support will be provided to assist with effective budgeting and where required open a bank account.

  •   Ensure service users’ freedom passes are up to date.

  •   Staff will engage with the individual on a daily basis as appropriate to support them to adapt to life in the community.

  •   Support with cleaning, tidying up and doing the laundry.

  •   Support individuals gain insight in food and hygiene, infection control, health and safety and fire hazard.

  •   Support people to use all appliances safely.

  •   People will be encouraged and supported to develop necessary skills in shopping for food and in the preparation of meals.

  •   The service will provide one meal per day with unlimited access to cereals, bread, milk, juice, frozen meals provided by the service.

  •   Prompting and support will be provided to improve skills in all aspects of daily living / self caring skills.

  •   Everyone will be encouraged to have a structured day and to take part in house and communal activities

  •   Prospective service users will be supported to take up educational opportunities, relevant training and/or voluntary work available locally.

Referral information

An up to date medical summary.

• A full risk assessment.

• Copies of recent C.P.A. meetings.

• Mental Health Act status and any expected changes, if applicable.

• We require relevant contact details for all those retaining responsibility for the resident during their stay with us. We also need emergency and out of hour’s contacts as well as those retaining responsibility for the resident post discharge.

• Where applicable, a copy of the conditions attached to a conditional discharge under Sections 37/41 of the Mental Health Act 1983.

• Information about informal contacts with family, friends or significant others with information on the quality of contact.

• An assessment of daily living skills.

• Information of the prospective residents’ strengths and assets areas where more support is required, their preferences and special needs or specific cultural requirements together with any activities enjoyed.

• A trial period shall last for a period of up to six weeks.

• Forward Community Care Ltd should receive written confirmation of funding for residence from the responsible authority. This includes funding for trial leave periods.

• Forward Community Care Ltd will ensure a thorough assessment is undertaken on each referral prior to admission.


Support Plan Intervention

  • Provided a safe group living therapeutic environment with a high level of support (24 hours including waking night) staffed by experienced mental health support workers managed by a team of mental health professionals.

  • Provide personal care

  • Each individual will have a comprehensive person-centred support plan in which they will be a full partner in the planning of. The plan will take in to account and respond to their support needs.

  • The aim of the plan will be to consolidate on progress made and to further develop independent living skills to attain an optimum level of mental health and functioning.

  • The prospective resident will have a key-worker who will engage with him and who will aim to develop a therapeutic relationship based on trust empathy and mutual understanding.

  • Their mental state and behaviour will be monitored with observations recorded and regularly evaluated.

  • Staff will work with the individual to support them to be fully compliant with his prescribed medication.

  • Staff will be aware of relapse indicators and appropriate responses.

  • Psycho education will be provided to gain insight in to their condition.

  • Support will be given to access Drug and Alcohol Specialist Service if required.

  • Drug screening and Breathalyser test can be provided.

  • Support will be provided to develop his self-esteem and confidence.

  • Everyone will have regular recorded key working sessions allowing time to express thoughts, feelings, anxieties etc.

  • Support will be given for service users to develop positive coping strategies.

  • Everyone will register with a local G.P., have access to a dentist, optician and other health professionals and be encouraged to attend his health checks and to follow medical advice given. The service will promote a healthy lifestyle.

  • Support will be given to maintain existing and develop new positive social contacts.

  • Encouragement and support will be provided for the residents to attend reviews and any external appointments. Escorts and house transport can be available.

  • Regular contact will be maintained with members of the clinical team where appropriate to monitor progress, mental state and review level of risk.

  • A contingency plan will be in place.
